Medical Release & Parental Consent Form - All students who are re-enrolling and have no changes to their medical history or insurance are not required to complete a new medical release. Others should complete a new form for 2026-2027.

Chelsea Tutorials Handbook - please complete the signature page and turn in only the signature page on August 18, 2026. You may print the handbook for your records.

There is a meeting for new students & parents on Tuesday, August 18h at 10am. All paperwork, supply fees, tuition and facility fees are due at this time.
